When my student attends a Christ Church Student Ministry event, what does she/he need?
  1. A current Contact and Emergency Care Information Form must be on file.
    Contact Form (complete one per family)
    Medical Emergency Care Form (complete one for each child)
    Forms may be completed and submitted electronically, or printed and returned to Julie Turner at the church. Please note: a copy of your medical insurance card must be on file. Please provide it via email or hard copy to Julie Turner.

     

  2. Completed Registration Form (and payment) for the event.
  3. Items listed on the registration form to bring.

Where do I get a medical release form?

Medical release forms can be found here and in the information rack inside the Student Center.

When and where do I turn in forms and money?
During regular programming hours, you can turn forms in to the Form Box in the Student Center.  Otherwise, please drop them off at the Receptionist Desk in a sealed envelope with Julie Turner’s name on the outside.

How strict are the deadlines to register for special events?
Deadlines are set because we need to make sure we have enough chaperones, housing, and transportation for each event.  If a form is turned in after the deadline, your student may miss out on the event.  We have registration forms for each event available at least 3 – 4 weeks before the event to ensure everyone has access to the form before the deadline.

What is Safe Sanctuaries?
Safe Sanctuaries is a policy designed to keep our children, students, and vulnerable adults safe.  Safe Sanctuaries is a background check for volunteers who come into contact with students on a regular basis. Volunteers must be active in the Christ Church congregation for at least six months before being allowed to supervise children or students.  If you are an active member of Christ Church and are interested in volunteering with Student Ministry, please fill out a Safe Sanctuaries application here.

As a parent, what is my role with Christ Church Student Ministries?
Parents are essential to this ministry! There are many ways to partner with us – chaperone a special event, volunteer to help with small groups or Sunday school – there are many opportunities. Contact Marc (HS) or Andrea (MS) to learn more.
