Welcome to the 2010-2011 registration page for Christ Church Children's & Student Ministries. This is your one-stop shop to register any and all children for ongoing programs at Christ Church! To find information and register for any programming offered to adults this fall, please click here!
- Medical Release Form - Please download the medical release form below, one for each child. You will need to fill out hard copies of each form for each child. Your child will not be able to participate in programming until we have an original form on file. Please hand your form to the receptionist, or if you prefer, you can sign the form, scan it into your computer, and e-mail it to Julie Turner, administrative assistant to age-level ministries.

- Safe Sanctuaries Form - Safe Sanctuaries background checks last for 3 years. If you have filled out this paperwork for last year, there is no need to do so again. If you have not filled out Safe Sanctuaries forms recently, please download a Safe Sanctuaries form below. All adults who wish to serve in some capacity with children and students must undergo a background check. Please fill out the form and submit it in a sealed envelope to Patty Groot, Director of Servant Ministries. You can hand the sealed envelope to the church receptionist or mail it to the church.
