Mini-Music Camp Registration

The One Voice Choir will host its first Mini-Music Camp June 4 - 5 from 6 - 8pm. The camp gives our One Voice students the opportunity to practice for the Music Camp they will host while on the 2012 Draw the Circle Wide Choir Tour and it will be a fun opportunity for Christ Church children to learn, sing and do musical crafts.
Children age three-years-old to fifth grade are invited to participate, please invite friends to join in the fun. There is no charge for the camp. Please bring a plain, white t-shirt.

Contact/Emergency Care Form

We need your Contact and Emergency Care Form for the 2011-2012 school year. The sharing of information essential to your child’s safety has been streamlined into one form, allowing you to provide information and give permission once for your family, as well as provide detailed information regarding any medical needs.  Every participating child, birth through age 18, will need a family Contact and Emergency Care Form on file.  All ministry areas will make use of the information you provide through this form.

Medical Emergency Care Form (complete one for each child)
Forms may be completed and submitted electronically, or printed and returned in a sealed envelope to a Children's Ministry staff member at the church. IMPORTANT: A copy of your medical insurance card must be included.
